He was predeceased by: his parents, Lewis Slade and Angeline Slade (Hodgin); and his siblings, Marilyn Hurd, Lewis Harry Slade and John Allen.
He is survived by: his children, Stephanie Putnam, Felicia Davis, Kimberly Slade and Timothy Slade; his grandchildren, Anthony Foster, Kegan Davis, Christopher Davis and Micheala Davis; and his siblings, Gloria Wilkins, Eileen Kimball (Midge), Karen Renn, Sandra Hammer, Beverly Ruszala, Margo Stark and Francis Allen.
