He was predeceased by: his parents, Maurice A. Cook and Beverly Cook (Stanhope); his sons, Colby and Caleb Cook; and his siblings, Cecil, Theresa and Robert.
He is survived by: his daughter Kayla Cook; his grandchildren, Elizabetta, Shailyn, Kenzi and Jackson; and his siblings, Mary Sanville (Dave), Kay Cook, Sandy Bushey (Tim), Maurice Cook, Linda Sweeney, Elaine Cook and Marjorie Wheeler (Ernie). He is also survived by many nieces and nephews.
