She was predeceased by: her parents, Aniello Formato and Carmella Formato (Tedesco); and her brother Joseph Formato. She is survived by: her husband Guido P. Forte Jr.; her children, Mark Forte (Libby), Gregory Forte (Terri), Karen Garrow (Stephen) and Julia Couture (Gerry); and her grandchildren, Marlee (Derek), Michael (Savannah), Mariah Forte, Elizabeth Alberico (Thomas), Kelsey Wenzel (Bradford), Joseph, Madison (Cameron) and Gabrielle Couture. She is also survived by several cousins, nieces and nephews.
