He is survived by: his wife Gail Lewis; his children, Peter Lewis and Robin Lewis (Emmett Sheppard); his grandchildren, Marley and Waylon; and his sister Darcelene (Michael Wedge). He is also survived by several nieces and nephews.
In lieu of flowers the family asks that you plant a maple tree in memory of David or make a donation to the Make-a-Wish foundation in his name.
