He is survived by: his parents, Michael Koontz and Michelle Koontz; his brother Lance Michael Koontz; his grandparents, Brent, Mary Koontz, Dee and Michael Black; his great-grandparent Carol R. Parker-Valparaiso; his aunts and uncles, Michael Black, Ben Koontz, Matthew Black, Amanda Black, Katie Koontz and Kathy Black; and his cousins, Brody, Kyle, Madlyn, Mikey and Mason.

Memorial contributions in Logan's name may be made to the Tabb High School Athletics Booster Club.
