He was predeceased by: his father Forrest Lee Hazelwood. He is survived by: his mother Ann Hazelwood; his siblings, Tiena Kusmira (Wayne) and Donald Hazelwood (Katherine); his nieces, Olivia and Holly Hazelwood; his nephew Landon Hazelwood; and his close friend Dorothy Mitchell. He is also survived by numerous cousins, aunts, uncles and friends.
In lieu of flowers memorial donations can be made to the American Heart Association.
