She was predeceased by: her parents, Bertha Easom Hazelwood and James Ivory Hazelwood; her husband Gerald Wayne Boaze; her son Jerry Wayne Boaze; and her brother Jimmy Hazelwood.
She is survived by: her children, R. T. Boaze (Danielle) of Sutherlin, VA, Sheri Stevens (Robert) of Sutherlin, VA and Teri Bray (David) of Keeling, VA; her siblings, Timmy Hazelwood (Linda) of Sutherlin, VA and Janice Simmons (Terry) of Keeling, VA; her grandchildren, Kayla, Ethan (Rebecca), Casey (Jess), Crissy, Bubba and Holden; her great grandchildren, Brayden and Clint; and her pet Ole Yellow. She is also survived by numerous nieces and nephews.
