He was predeceased by: his parents, James Ivory Hazelwood and Bertha Easom Hazelwood. He is survived by: his wife Janet Whitley Hazelwood; his children, Lori H. Ingram (Gordon) of Blairs, VA, Thomas C. Hazelwood (Lisa) of Palmyra, VA, Michael A. Hazelwood (Peggy) of Sutherlin, VA, Jason V. Hazelwood (Teresa) of Ringgold, VA and Jeffrey R. Morrow (Robyn) of Logan, Utah; and his siblings, Alma Boaze, Janice Simmons (Terry), Timothy M. Hazelwood (Linda) and Steve Crabb (Barbara). He is also survived by twelve grandchildren and numerous great grandchildren.
