He was predeceased by: his father Charles Lewis Cook Jr.. He is survived by: his mother Lena Mahulda Cook (Bright); his wife Julia Marie Lockridge; his son Samuel Lewis Cook, II (Keri) of Stuarts Draft; his siblings, Charles "Chip" Cook, III of Verona, Scott Cook (Helen) of Port Republic, Robin Painter (Robert) of Stuarts Draft and David Cook (Janet) of Stuarts Draft; his granddaughter Hailey Cook; his nephew Shaun Baker; his nieces, Jessica Baker and Hope Cook; and his pet Merlin.

In lieu of flowers, the family requests all memorial contributions be made to the Blue Ridge Area Food Bank, P.O. Box 937, Verona, VA 24482-0937.
