He was predeceased by: his parents, Thomas Middlebrooks and Jeanette Middlebrooks. He is survived by: his wife Juanita; his children, Sherry, Thomas, Douglas and Benjamin; his grandchildren, Breighanne, Kesleigh, Delaini, Savannah, Danny, Everett and Anastasia; his great grandchildren, Emery, Leonardo, Genevieve and Chase; and his siblings, Matt Middlebrooks and Jennie Welsh.
