She was predeceased by: her mother Nancy Sullivan. She is survived by: her father Jerry Hayes; her husband Michael LaCasse; her children, Chloe Faller (Thomas), Kayla and Tyler; her siblings, Jerry Hayes (Lisa) and Denise Hayes; and her nieces and nephews, Kyle (Courtney), Kelly, Melanie, Katie and Christian. She is also survived by her family, friends.
