He was predeceased by: his parents, John "Blaze" Cook and Gladys Carter Cook; his wife Mathelda A. Cook; and his brothers, James Cook, Bennie Cook, John "Timpy" Cook and Clarence Ellis.
He is survived by: his caregiver Consumer Direct; his sons, James "Larry" Banks (Terri) of Woodbridge, VA, Elmo Cook (Francine) and Kevin Cook (Shelia) of Spotsylvania, VA; his grandchildren, Cortez Cook (Morgan), Caleb Cook, Keturah Cook and Tabitha Cook; and his brother Jenis Ellis (Sandra) of Spotsylvania, VA. He is also survived by brothers-in-law, sisters-in-law; and a host of nieces, nephews, cousins, and other relatives.
