He is survived by: his wife Cynthia Foor Callis; his children, Sara Williams (Brandon), Meaghan Koehler (Derek) and William Callis; his grandchildren, Hannah, Madalynn and Clayton; and his sister Faith Callis Blake (Alan). He is also survived by 7 nieces and nephews.

In memory of Jack, contributions may be made in his name to Reedswood Christian Church or the American Cancer Society.
