He was predeceased by: his father John Charles Horn, Jr.. He is survived by: his mother Gail Bruffy (Ernie); his wife Donna Creasy Horn; his children, Ryan Horn (Kaitlin), Blake Horn, Emma Horn, Crystal Falls and Carrie Falls; and his sisters, Lori Webb (Mike) and Lisa Moore (Tommy). He is also survived by five grandchildren.
