He was predeceased by: his wife Ann Lee Alexander Cook; and his parents, Chester V. Cook and Maybelle Ruth Edgecomb Cook. He is survived by: his daughters, Elizabeth Shannon Cook Jeans (Robert C. Jeans) of Glade Hill and LeeAnn Alexander Cook (Christopher Horn) of Rocky Mount; his grandchildren, Rachel Justine Mathena (Brandon Mathena), Elizabeth Ann Alexander Jeans and John Berger Jeans (Hannah); his brother Theodore Malcolm Cook (Sarah Cook) of Prescott, Washington; and his brother-in-law Judge William N. Alexander (Sarah Davis Alexander). He is also survived by nieces and nephews.
