He was predeceased by: his parents, Donnie D. Altizer and Virginia L. Altizer; and his brother Paul Wayne Altizer. He is survived by: his wife Phama Altizer; his son Josh; his daughter-in-law Brandy Altizer; his daughter Tara; his son-in-law Adam Lephew; and his grandson Avery Lephew.
