He was predeceased by: his parents, Ed Cook and Mary Addison Cook; his wife Betty Lou Edwards Cook; his daughter Charlotte Cook Mackey; and his brothers, Bobby and Perry Cook.
He is survived by: his daughters, Jennifer Cook Lowe (Ernest) of Oakwood, VA and Tara Cook Hill (Eric) of Lebanon, VA; his siblings, Nell Hurt (Earl), Ann Cook, Bill Cook (Billie) and Jack Cook (Betty); his grandchildren, Victoria Keen, Derek Keen (Deanna), Whitney Denver of Logan Mackey, J. D, Lesly Roope, Carter Hess, Knox Mitchell, Madison Hill and Brayden Hill; and his great grandchildren, Kadence Keen, Austin Keen, Grayson Lockhart, Kameron Keen, Brayden Denver, Aubree Denver, Isabella Pruitt, Lukas Denver and Jaxson Lowe. He is also survived by nieces and nephews.
