He was predeceased by: his parents, Mary Elizabeth Wyatt Dalton and Paul Revele Lewis; his wife Pat; his siblings, Carolyn Fletcher and Noel Lewis; and his grandson Bryce Lewis.
He is survived by: his children, Pam Lewis Hash (John), Dale Edwin Lewis Jr. (Angie) and Missy Lewis Martin (Tim); his sister Janice Doherty; and his grandchildren, Sarah (Alec), Katie (Michael), Celeste (Chris), Frank, John III (Moya), Claire, Kalynn (Nick), Grace, Anna and Luke. He is also survived by his children, grandchildren and great-grandchildren.
