He was predeceased by: his parents, Henry Stephen Forrest, Sr. and Edna L. Saunders Forrest; his brother H. S. Forrest, Jr. (Roselyn); and his great-niece Mary Ann Forrest Rector. He is survived by: his wife Lois Hall Forrest; his daughter Dana Forrest; his son-in-law Joseph M. Zawodny; his granddaughter Eva C. Zawodny; his son Orville H. Forrest Jr. (Butch); his nephews, Daniel W. Forrest (Betty) and H. Stephen Forrest, III (Kay); his great-niece Kelly; and his great-nephews, Cleveland, Matthew (Amy) and Carson.
