He was predeceased by: his parents, Geraldine Blevins and Benjamin Blevins. He is survived by: his wife Anita Darlene Blevins; his son David Blevins, II; his wife Traci Blevins; his siblings, Richard Blevins (Nancy), Kenneth Blevins and Shirley Blevins; and also Daniel Blevins (Chasidy).

In lieu of flowers the family suggests that memorials be made to St. Jude Children's Hospital.
