He was predeceased by: his parents, John Ingram Hazelwood, Sr. and Virginia Mae Blankenship Hazelwood; his son Jeffrey Lee Hazelwood; and his siblings, Ronald "Ronnie" Hazelwood, Thomas "Corky" Hazelwood, Monty Ray Hazelwood and Bonnie Mann.
He is survived by: his wife Patty Smith Hazelwood of Narrows; his son Jess Robert Hazelwood (Karen) of Gloucester; his brothers, Ricky Hazelwood of Lindside, WV and Joe Hazelwood (Linda) of Lindside, WV; his daughter-in-law Susie Bergsten (Kelvin) of Blacksburg; his grandchildren, Katima Farrow of Richmond, Jessica Roane (Jacob) of Gloucester and Kursten Smith (Kenny) of Gloucester; and his great grandchildren, Aiden, Ava and River.
