He was predeceased by: his parents, Harley Hershel Heath and Lela Gertrude Heath (Pepper). He is survived by: his spouses, Maria Makowelski Heath and Harley Hunter Heath; his siblings-in-law, Alexander Makowelski Jr. (Lisa), John James Makowelski, Catherine Gray (Victor), Jane M. Staples (Steve) and Elizabeth A. Schneider (Herby); and his son Harley. He is also survived by son, he is survived by his brothers.
In lieu of flowers, memorial contributions may be made to the family, in Choo's honor, for his son Harley.
