He was predeceased by: his wife Hazel R. Cook; and his parents, Ralph C. Cook Sr. and Ethel Cook. He is survived by: his children, Beverley Evans, Randy Cook (Wanda), Ronnie Cook (Virginia) and Robin Butler (Rick); his grandchildren, Stephanie Evans, Tabitha Murphy, Stephen Cook, Jennifer Keener, Jason Cook, Aaron Cook, Megan Butler, Claire Bucci and Peyton Butler; and his sister Catherine Elliott. He is also survived by 6 great grandchildren; two great-great-grandchildren.
In lieu of flowers, donations may be made to the Endowment Fund at Parham Road Baptist Church.
