She was predeceased by: her father Spencer T. Hazelwood Jr.; and her mother-in-law Charlotte Hood Alexander. She is survived by: her husband Sanford "Miles" Boutchyard; her mother Betty Layne Hazelwood; her siblings, Jennifer H. Trent (Jesse Baker) and Ben Hazelwood (Alice); her cousin Dana Walters (Ben); her goddaughter Taylor Douglas; her father-in-law Jerry Alexander; her brothers-in-law, Mike and Jesse Boutchyard; her nephew Dillon Boutchyard; and her daughters, Cassidy and Sandy.

In lieu of flowers, memorial contributions may be made to the Hanover County Animal Control and Shelter, 12471 Taylor Complex Ln., Ashland, Va. 23005.
