He is survived by: his wife Mary Stack; his parents, Peter Stack and Rosalie Tarnowski Stack; his grandson Daniel Edward Stack; his great grandchild Easton Roy Shepherd; his children, Jerome D. Stack Jr. (Susan), Susan S. Wawrzyniak (Karl) and Michele S. Martin (Drew); and his siblings, Thomas Stack and Patricia De. He is also survived by 12 grandchildren; 4 great-grandchildren.
