She was predeceased by: her husband Richard Fontaine; and her parents, Lillian Slater, and Edward Slater, Jr.. She is survived by: her sons, Michael Fontaine, James Fontaine (Jody) and David Fontaine (Sandra); her siblings, Betty Flippo (Harold), Jane Madison and Ed Slater; her friend Rick Miller; and also, Juanita and Susan. She is also survived by five grandchildren and many great-grandchildren.
In lieu of flowers, donations can be made to At Home Care Hospice in appreciation of their kindness and care during Ann's final days.
