He was predeceased by: his parents, Carlis Lewis and Eula Clark Lewis. He is survived by: his wife Patricia M. Lewis; his children, Patricia Faye Lewis, Angela Gail Lewis-Childress, Nancy Ann Lewis-Robinson (Christopher) and Addison Wayne Lewis (Gayle); and his grandchildren, Ashley Childress, Garrett and Everett Robinson.

The family requests that in lieu of flowers, memorial contributions be made to Oakland, U.M.C.
