He was predeceased by: his parents, Ivan Onufrak and Pelagia Onufrak (Suchy); his wives, Christine (Danchak) and Mary (Baysa); and his brothers, Michael, Joseph and Andrew.
He is survived by: his children, Donna Smith of Edinburg, VA, John Onufrak of McLean, VA and Patricia Onufrak of Falls Church, VA; his sisters, Ann Adams of Lancaster, PA and Helen Onufrak; and his sisters-in-law, Dorothy Baysa of Browndale, PA and Mary Ellen Danchak of Clayton, NC. He is also survived by six grandchildren, three great-grandchildren, and numerous nieces and nephews.
