He is survived by: his wife Marvie McCain Cook; his children, Michael D. Cook, Jr. (Helene Brown) of Jacksonville, FL and Michelle D. Adams (Joshua) of Fernandina Beach, FL; and his siblings, Jackie Mosley of Greensboro, NC, Penny Parker of Charlotte, NC, Ricky Cook of Greensboro, NC and Marvin Cook (Leslie) of Lithonia, GA. He is also survived by nieces, nephews, other relatives, and friends.
