He is survived by: his sons, Zachary Holzworth (Mary Catherine Brown) and DW Holzworth, Jr. (Lauren Hall); his parents, Jesse Fitzgerald (Pops) and Bonnie Fitzgerald; his brother Jim Holzworth; his niece Marisa Holzworth; his great-nephew Michael Strumb; and the mother of his children Leigh Ann Forquer.
