He was predeceased by: his father Willard Douglas Cook; and his son Brandon Cook. He is survived by: his children, Andrew Collins, Mollie Collins and Madison Cook; his mother Martha Marie Hall Cook; and his siblings, Michael Cook (Greta), Willard Cook, Jr., Wilma Hurd (John) and Diana Helton (Roger).
