He was predeceased by: his parents, Charles Lively (Charlie) and Christine Lively; and his sister Debbie Lively. He is survived by: his wife Susan; his children, Jessica Simmons (Coleman) and Travis Lively (Kaitlyn); and his grandchildren, Easton, Calvin Lively, Adeline and Everlee Simmons. He is also survived by numerous family members and dear friends.
