He was predeceased by: his parents, Issac Levi Bodenhamer and Dora Floe Cook; his brothers, Jack Miller, Howard Miller, Mack Cook, David Cook and Wendell Cook; and his son Gregory Alan Cook.
He is survived by: his wife Barbara; his children, Gena Keller (Larry), Donna Crider (Keith), Dale Cook (Julie) and Scott Cook; his siblings, Hava White, Ted Cook (Greta), Bill Cook (Marie), Wanda Baker and Barbara Leake; and his great grandchildren, Madelyn, Rory, Lucas, Noah, Elaina, Eli, Chase, Caleb and Then. He is also survived by 12 grandchildren and 8 great grandchildren.
