Forman (Fredericksburg, Virginia), born in Rochester, New York, whose presence will be deeply missed, having departed on September 19, 2024 at the age of 69. He was predeceased by: his parents, Winfield Scott Forman, Sr. and Ednamaye Dickenson Forman; and his siblings, Charles, James, Robert, Winfield, Jr. and Beverly.
He is survived by: his wife Lori; his children, Jason, Justin and Thomas II; and his brother Michael. He is also survived by numerous cousins, nieces, and nephews.
