He was predeceased by: his father Darrell Cook; and his brother Danny Matthew Cook. He is survived by: his wife Terri L. Cook; his sons, Danny "Matt" Cook, II (Jessica) and Scott Johnson (Jennifer); his granddaughters, Skylar Grace Cook and Summer Alexandra Byars; his mother Mary Cook of Virginia; and his siblings, Dennis Michael Cook (Jackie), D. Hope Seckinger (Stephen) and Donna Marie Cook.
