He was predeceased by: his parents, Charles Henry Ballard and Mary Deitz Davis Ballard; his wife Doreathea Ballard; his son Jim H. Ballard; and his siblings, Eva Reynolds, Myrtle Shumate, Violet Stegall, Gertrude Hopkins, Ruth Foley, Alma Shatzar, Dewey Ballard, Ralph Ballard and Wayne Ballard.
He is survived by: his daughter Rachel Brooks (Roger); his daughter-in-law Gale F. Ballard; his grandchildren, Robin Sawyers (Roy), Tiffany Ballard and Kristie Brooks; his great grandchildren, Austin Crawford, Alicia Gusler and Rachael Potter; his great great grandchild Mason Lee Gusler; and his siblings-in-law, Elsie Haynes and Druie Martin.
