He was predeceased by: his father Theodore Butterworth; and his daughter Jill Butterworth. He is survived by: his mother Donna Heater Butterworth; his son Jeremy Butterworth (Brandy Dowell); and his siblings, Joel Butterworth and Judy Butterworth. He is also survived by four grandkids.
