She was predeceased by: her parents, Jesse Milton Mathias and Bessie M. Lonas Mathias; and her siblings, Derwood (Thelma Mathias), Jerry Mathias and Annetta Lambert.
She is survived by: her husband Leroy "Cook" H. Polk; her daughter Elisabeth Ann Loy of Winchester; her grandson Bruce Green of Seattle, Washington; and her brother Elroy Mathias (Pat) of Winchester. She is also survived by nieces, nephews, cousins.
