He was predeceased by: his wife Vilda L. Mathias Polk; his parents, George T. Polk and Ida Dellinger Polk; his siblings, Leo T "Bud" Polk, Sr., Polly P. Hammand, Gladys P. Dellinger, Georgia P. Bowers and Ethel P. Spiggle; and his nephews, Stanley O. Dellinger and Roscoe Bowers.
He is survived by: his daughter Elizabeth Ann Loy of Winchester, Virginia; his grandson Bruce Green of Seattle, Washington; his siblings-in-law, Elroy and Patricia Mathias of Winchester, Virginia; his nephew Leo T. "Buddy" Polk; and his nieces, Bonnie Andes, Kay Hamman and Erma Baker. He is also survived by great nieces, great nephews.
