He was predeceased by: his parents, Dorothy Cook and Gordon Cook; and his sisters, Lois Daniel and Teenie Cook. He is survived by: his wife Gail Hammonds Cook; his daughters, Amy Cook (Michael Goodman) and Ashley Cook (Carlos Hernandez); his grandchildren, Eric, Alanna Cook and Charlie Hernandez; and his siblings, Bertie Cook of Crewe, Bob Cook of Crewe and Billy Cook of Chesterfield.
