He was predeceased by: his parents, Linda Stevens Moxley and Jack Randall Moxley; and his sister Rhonda Lynn Moxley. He is survived by: his wife Susan Marie Webb Moxley; his children, Sabrina Lea Moxley, Jesse Thomas Moxley and Johnny O'Neil Moxley; and his granddaughter Melody.
