He is survived by: his wife Jody White Forrest; his daughters, Hillary Forrest-Wood (James) and Jennie Forrest; his grandchildren, Casey Elliott, Jonathan, Jakson Kurtz, Andrew Jackson Hensley and Kynleigh Wood; and his siblings, Bonnie Bruno, Cindy Price and Joseph Forrest.
