He was predeceased by: his grandparents, Charles, Goldie Sloan, Danny and Betty Johnson; and his mother-in-law Eulonda Cash.
He is survived by: his parents, Tommy Johnson and Connie Sloan Johnson; his wife Melinda Cash Hayslett; his children, McKenna Shane Hayslett and Cane Winsom Hayslett of Eagle Rock; his brother Jeremiah Johnson of Roanoke; his aunt Doris Balser; and his fathers-in-law, Bobby Eisenhower and Ronald Cash. He is also survived by aunts, uncles and cousins.
The family suggests memorial tributes take the form of contributions to Clifton Forge Food Pantry, PO Box 351, Clifton Forge, Virginia 24422.
