He was predeceased by: his parents, Judge Dirk A. Kuyk and Mary Terry Goodwin Kuyk. He is survived by: his son Dirk III; and his sister Martha Kuyk Hull.

Instead of flowers, the family asks that contributions be made to Trinity College, 300 Summit St., Hartford, CT, 06106.
