He was predeceased by: his mother Eva; his stepfather Donald; and his brother Otho. He is survived by: his wife Tammy Lynn Butler; and his children, Angela, Ashley, Kristan, Shawn and Tasha. He is also survived by his grandchildren, and great-grandchildren.
