He is survived by: his sons, David Cook (Julie) and Ricky Truelove; his brothers, Paul Cook (Irene) and Curtis Cook; his grandchildren, Marissa, Kari, Analesia, Brooklyn, Brianna, John and Zach; and his great grandchildren, Skylar, Colton, Cody, Bentlee, Gabriel, Ariel, Zeke and Cassie.
