He is survived by: his parents, Alexander Mackay-Smith, Jr. and Leigh R. Mackay-Smith of White Post; his children, Gillian Mackay-Smith of Essex, MA and Hillary Mackay-Smith of Ipswich, MA.; his grandchildren, Isabel Klein, Isla Klein, Hannah Klein and Lucas Chonde; his sons-in-law, Dr. Daniel Chonde of Essex and Matthew Klein of Ipswich; and his siblings, Alexander Mackay-Smith IV (Nahomi) of Stephens City, VA, Catharine Kempson (Kenneth) of White Post, Virginia Mackay-Smith of White Post, Anne Vance (Terry) of Sharon, CT, Alexandra Keirstead (James) of Stephens City, Helen Mazarakis (Thanassis) of Montclair, NJ and Barbara Mackay-Smith (Michael Yoder) of Novato, CA. He is also survived by many nieces, nephews, and cousins.

A funeral service was held on Friday, February 7th 2025 at 2:00 PM at the Meade Memorial Episcopal Church (192 White Post Rd, White Post, VA 22663).
