He was predeceased by: his parents, Raymond Edward Stevens and Beulah Bates Stevens. He is survived by: his wife Crystal "Penny" Burkholder Stevens; his children, Kathy Mayhew (Ronny) and Jason Stevens (Stephanie); his grandchildren, Isaak Mayhew, Lydia Mayhew, Kaylin Stevens and Jacob Stevens; and his brothers, Phil Stevens (Millie) and Lewis Stevens. He is also survived by numerous nieces and nephews; and his church family.
