He was predeceased by: his mother Donna Marie Romer Hazelet; and his grandparents, James, Hazel Hazelet, Robert and Jacqulynn Romer.
He is survived by: his father John Scott Hazelet Sr. of Bedford; his brother John Hazelet Jr. (Jessica); his children, Jordan Hazelet and Jason Hazelet, Jr.; his aunt Jan Gravante; his nephews, John Hazelet III (JT) and Joseph Howell; and his great-nephew Lucas Hazelet. He is also survived by numerous other aunts, uncles and cousins.
