He was predeceased by: his father George Robert Kokochak. He is survived by: his mother Edna Barnhill Kokochak of North Carolina; his wife Jennifer Katherine Kokochak of Nevada; his sister Alta "Bunny" Kokochak of North Carolina; his half-sister Joyce Sample-Franco of Michigan; his niece Katherine Heather Kokochak of Texas; and his great-nephew Wyatt Liam Moore of Texas. He is also survived by his family and friends.
